Beyond Total Cholesterol Six Vital Markers for a Clearer Picture of Heart Health

Dr. Dmitry Yaranov, a cardiologist and heart transplant specialist, asserts that relying solely on total cholesterol numbers provides an incomplete picture of cardiovascular risk. While total cholesterol offers a broad overview, it fails to distinguish between the various particles that actually drive arterial plaque buildup. To gain a more accurate assessment, Dr. Yaranov emphasizes monitoring LDL-C (the primary driver of plaque) and ApoB, which counts the total number of harmful, plaque-forming particles in the blood. He notes that if LDL and ApoB levels conflict, the ApoB reading is the more reliable predictor of heart disease risk.

In addition to these primary markers, the cardiologist highlights the importance of tracking Non-HDL cholesterol as a practical backup and monitoring triglycerides, which can signal an excess of harmful particles when elevated. A critical, often overlooked factor is Lp(a), a genetically inherited lipoprotein that should be measured at least once in a lifetime, as it can significantly increase risk even when other numbers appear normal. Finally, Dr. Yaranov cautions that while high HDL (“good” cholesterol) is beneficial, it does not “cancel out” the dangers of high LDL. By focusing on lowering ApoB and LDL while understanding one’s genetic Lp(a) levels, individuals can take more precise control over their long-term heart health.
